TRYOUTS – Health Form Requirements

All Student-Athletes must use Arbiter Registration to download all Tryout Health Requirement Forms.

Annual Physical Form
All Students must submit a valid Annual Physical Report before they can try out.
Physicals are valid for 13 months after the Physical Examination Date.

Concussion Awareness Training Form
All Students must take the required Concussion Awareness course before they can try out.
To ensure continued safety for all students participating in Athletics at the high school level, the state of Massachusetts, along with the MIAA, have recently mandated that all student-athletes, as well as parents, must participate in the National Federation of State High School Association’s Concussion Awareness program.

This program consists of online courses.  Students must take the "Concussion for Students" course while a Parent must take the “Concussion in Sports” course.  There is a corresponding exam for each course. These courses can be found at www.nfhslearn.com and are free.  All Students and a Parent/Guardian will be required to participate in these online courses. The student and a parent should create their own separate accounts to secure certificates in their own name. 

No student will be allowed to participate in any Athletic practice, scrimmage, or game at Saint John’s High School without downloading both his and a Parent/Guardian’s Certificate of Course Completion.
 
Pre-Participation Concussion Form
All Students must complete the Pre-Participation Concussion Questions on Arbiter Registration before the start of every Athletic season for which they plan to participate. 
In keeping with the requirements set forth in the Massachusetts 105 CMR 201.000, this form must be completed by a student’s parent(s) or legal guardian(s).

Overview
Lacrosse has been played competitively at St John's since the 1980's. It is one of the fastest growing sports at St. John's and in high schools across the country. It attracts some of the school's best student athletes, and the coaching staff encourages students to try out for the sport even if they have little or no experience. Lacrosse is a fast-paced game which incorporates many skills found in other sports, thus making it easy for a good athletes to pick up.


Practices
The teams practice every day after school from 3:00 - 5:30 pm, and on most non-game Saturdays. Typically, games are not scheduled for the Freshman team over the April Vacation.

Transportation
Buses are provided for away games.


Equipment
Students are required to purchase all of the equipment necessary to play lacrosse
  • Helmet, Stick, Gloves, Shoulder Pads, Elbow Pads

Physicals
An annual physical is required to participate. Please be aware that physicals are only good for 13-months and the lacrosse physical must be good as of 20 Mar 2017 to cover tryouts. Once the physical expires an updated one will be required.
  • Players must fill out a Pre-Participation Form (for every sports season)
  • Players must complete Concussion Training (once per school year)
